Participate Assistance & Care (PAC)
PAC services enable individuals with intellectual or developmental disabilities to live successfully in their own homes, participate in their communities, and avoid institutionalization. These services provide hands-on assistance, prompting, monitoring, and support for daily living activities to ensure health, safety, and well-being.
Key Features of PAC Services:

Assistance with personal care, meal preparation, shopping, errands, scheduling appointments, and leisure activities (excluding transportation).

Support for mobility, including transfers, ambulation, and the use of assistive devices.

Help with correspondence, bill-paying, and other administrative tasks.

Escorting individuals to community activities and appointments.

Monitoring, reinforcement of behavioral supports, and adherence to risk plans.
Available individually or as a shared service, PAC focuses on supporting independence while addressing the unique needs of each individual.

Community Based Day Habilitation (Individual & Group)
Community Day Habilitation services focus on helping individuals with intellectual or developmental disabilities acquire and retain skills in areas such as self-care, socialization, daily living, communication, and community living. These services aim to foster independence, build relationships, and create natural supports.
Key Features of Day Habilitation Services:

Activities are offered in community settings or non-residential facilities approved by FSSA/DDRS, separate from the individual’s home.

Training and support for engaging in leisure activities, community/public events, educational programs, hobbies, and unpaid work experiences like volunteering.

Assistance in maintaining relationships with family and friends.

Education in self-direction, including developing self-advocacy skills, exercising civil rights, and taking responsibility for services and supports.

Skill-building to promote greater independence, integration, and productivity in the community.
Day Habilitation is customized based on the Person-Centered Individualized Support Plan (PCISP) to help individuals achieve their personal goals and enhance their quality of life.
